Operations advisory

Your best people are doing admin work.

Collina helps U.S. medical practices and law firms find where the hours are leaking, fix the workflows underneath, and hand the whole thing back to your team documented.

Two weeks
From first call to a written plan you can act on.
4× ROI
Guaranteed on the Operations Audit, or we refund it.
Two sectors
Medical practices and law firms. Nothing else.
— Engagement formats

Three ways we can work together.

— A typical first 90 days

From inquiry to installed system.

Week 0

Discovery call

30 minutes, no deck. We listen, identify the highest-leverage opportunity in your operation, and tell you whether we're the right firm to help — or who is.

Weeks 1–2

Operations Audit

We embed, observe, and time-stamp every workflow. By end-of-week-two, you have a written report and a sequenced action plan with hard-numbered savings.

Weeks 3–8

Automation Build

We build the highest-ROI fixes from the audit. Most clients see the first quantifiable time-savings inside the first three weeks of build.

Weeks 9–12

Handover & baseline

Your team runs the new systems. We measure the delta, hand over the documentation, and remain on call for thirty days. Most clients then move into Retainer.

Month 4 onward

Retainer Advisory — optional

For clients who want an ongoing operating partner on call. Month-to-month, capped roster, senior-led.

— Selected case notes

A few engagements, anonymised.

— Surgical Practice · TX

Deposition coordination eating clinical time

A specialist surgeon spending 6+ hours weekly on cross-firm scheduling, invoice follow-up, and document requests. We mapped, automated, and templated the entire law-firm correspondence cycle.

Outcome
5.5 hrs/wk returned to clinical
— PI Firm · FL

Intake leakage and partner-level admin

A growing personal-injury firm losing qualified intake leads to slow follow-up. Partners doing first-touch correspondence at 11pm. We built triage, routing, and a structured intake response system.

Outcome
+22% intake conversion, Q1
— Specialty Clinic · CA

Invoice cycle slipping to 60+ days

A multi-physician specialty clinic with an invoice-to-paid cycle drifting past 60 days. We installed a tight follow-up cadence, documented the chase protocol, and trained the front-desk team.

Outcome
34-day avg. invoice cycle
Identities anonymised by client agreement. Full reference calls available on request, post-NDA.

— Frequently asked

The questions every operator asks.

Most work is done remotely. Morning hours in Sweden overlap with the U.S. East Coast, which is usually when clients have time to talk anyway. When it's genuinely useful to be onsite, we travel.
Both sectors have the same underlying problem: highly-paid professionals doing low-value admin work, with nothing underneath them to absorb it. Both bill by the hour, so lost time is lost money in a very direct way. We've worked inside this exact problem on both the medical and legal sides — it's what we know best, so it's what we focus on.
Solo practitioners up to firms of about 25 people. Smaller than that, an Audit is probably more than you need — we'll point you to something simpler instead. Larger than that, it still works, but the scope and price grow with it.
Almost never. Most practices already pay for tools that can do most of what they need — they're just not set up right. We'd rather make what you already own work harder than sell you something new. If new software is genuinely the right call, we'll show you a maximum of three options and let you choose. We don't take referral fees from vendors.
A fractional COO is a hire — usually expensive, near full-time, and embedded for a year or more. A regular consultant often hands you a slide deck and disappears. Collina sits between the two: we do the work ourselves, install it, document it, and hand it to your team — with ongoing advisory available after, if you want it. We're not trying to make ourselves indispensable. We're trying to make ourselves unnecessary, then stay close in case you want a second opinion.

Thirty minutes. Then you'll know.

Book a call and we'll tell you plainly: whether your back office has the kind of problem we fix, what the biggest opportunity is, and whether we're the right firm to help. If we're not, we'll say so.
